Abstract

Analizar la implementación de un programa preventivo de cáncer de mama en un financiador privado de salud mediante mamografía digital en mujeres mayores de 40 años con seguimiento, comparándolas con un grupo histórico sin dicha intervención. Métodos: Estudio observacional analítico transversal retrospectivo que comparó los períodos pre y post aplicación del programa. Se evaluaron edad, año y tipo de diagnóstico, clasificación BIRADS, fechas de estudios, confirmación histopatológica e inicio de tratamientos (cirugía, radioterapia, quimioterapia), además de los tiempos de espera entre prestaciones. Resultados: Se incluyeron 688 registros (562 del programa preventivo y 126 históricos). La pesquisa precoz incrementó significativamente la detección en estadios tempranos (BIRADS 0 a 3) frente al grupo no preventivo (100% BIRADS 4). No hubo diferencias significativas en los tiempos de espera entre el diagnóstico histopatológico y el inicio del tratamiento, pero sí se observó una reducción estadísticamente significativa en la letalidad a favor del grupo bajo el programa preventivo.

To analyze the implementation of a breast cancer prevention program by a private health insurer using digital mammography in women over 40 with follow-up, comparing them with a historical group without such intervention. Methods: A retrospective cross-sectional analytical observational study comparing the pre- and post-application periods of the program. Variables evaluated included age, year and type of diagnosis, BIRADS classification, dates of studies, histopathological confirmation, initiation of treatments (surgery, radiotherapy, chemotherapy), and waiting times between services. Results: A total of 688 records were included (562 from the preventive program and 126 historical controls). Early screening significantly increased detection in early stages (BIRADS 0 to 3) compared to the non-preventive group (100% BIRADS 4). No significant differences were found in waiting times between histopathological diagnosis and treatment initiation, but a statistically significant reduction in mortality was observed in favor of the group under the preventive program.
