Caracterización de la atención en guardia del SAMCo Sunchales en el primer semestre 2023-2024

Abstract

El presente Trabajo Final Integrador, desarrollado en el marco de la Carrera de Especialización en Auditoría Médica de la UNL, tuvo como objetivo caracterizar la atención brindada en el servicio de guardia del SAMCo Sunchales durante el primer semestre de los años 2023 y 2024. Se realizó un estudio analítico, observacional y retrospectivo, basado en el análisis de datos obtenidos de los sistemas informáticos de registro SICAP y Diagnose. Se evaluó el número total de consultas por guardia, la proporción de pacientes con y sin cobertura de obra social o medicina prepaga, la recaudación generada por las contraprestaciones brindadas, los costos del servicio de guardia y los principales motivos de consulta. Los resultados mostraron un aumento significativo en el total de atenciones durante el primer semestre de 2024 respecto del mismo período de 2023, así como un incremento en la proporción de pacientes con cobertura, que pasó del 8,22% al 11,41%. Sin embargo, la recaudación obtenida no presentó una relación proporcional con el aumento de las prestaciones, resultando insuficiente para cubrir los costos del servicio, especialmente las retribuciones médicas, en un contexto de elevada inflación. Asimismo, se observó que la mayoría de las consultas correspondieron a patologías de bajo riesgo, potencialmente resolubles en el primer nivel de atención. Se concluye que el incremento de la demanda en la guardia del SAMCo Sunchales pone de manifiesto la necesidad de optimizar la organización del sistema, fortalecer la atención primaria y mejorar los mecanismos de recupero económico desde la perspectiva de la auditoría médica.

This Final Integrative Project was developed within the framework of the Medical Audit Specialization Program at the UNL and aimed to characterize the care provided in the emergency department of SAMCo Sunchales during the first semester of 2023 and 2024. An analytical, observational, and retrospective study was conducted based on the analysis of data obtained from the SICAP and Diagnose information systems. The study assessed the total number of emergency consultations, the proportion of patients with and without social security or private health insurance coverage, the revenue generated through billed services, the costs associated with emergency care, and the main reasons for consultation. The results showed a significant increase in the total number of emergency visits during the first semester of 2024 compared to the same period in 2023, as well as an increase in the proportion of patients with health insurance coverage, which rose from 8.22% to 11.41%. However, the revenue obtained did not show a proportional relationship with the increase in services provided and was insufficient to cover the costs of the emergency department, particularly physicians’ remuneration, within a context of high inflation. Additionally, a large proportion of consultations corresponded to low-risk conditions that could potentially be managed at the primary care level. It is concluded that the sustained increase in demand for emergency services at SAMCo Sunchales highlights the need to optimize service organization, strengthen primary health care, and improve financial recovery mechanisms from a medical audit perspective.
